ATTEND AND ORIENTATION THE 1^ST TUESDAY OF THE MONTH (EXCEPT JANUARY) AT 9AM

DOCUMENTS TO BE SUBMITTED FOR APPRENTICESHIP

1. Fill out the Pre-Trial Apprentice Application form in its entirety. (Application provided at orientation)
2. You must furnish with your application with the following:
   1. $50 Applicant fee (cash, check, money order or card – payable to Mid-America Carpenter Training Center)
   2. A copy of your birth certificate
   3. A copy of your high school diploma or GED
   4. A copy of your high school transcripts (regardless of how long you attended high school)
      1. The high school can email the transcripts
      2. If applicable – college transcripts, certifications, or DD214
   5. A resume or work history
   6. 4-5 Letters of Recommendation (can be from anyone)
   7. IH Mississippi Valley CU application w/ copy of Driver’s License

**To get  working – you are required to have the following submitted:

• $50 Applicant Fee
• Application
• Copy of Birth Certificate
• Copy of High School Diploma or GED

** If you attended an orientation January – April …. Then ALL documentation is due no later than April 30^th  of that year, or you will not be invited to the Math Test in May

** If you attended an orientation May – July … Then ALL documentation is due no later than July 31^st of that year, or you will not be invited to the math test in August**

**** If you attended an orientation Aug – December … Then ALL documentation is due no later than January 31^st of that year, or you will not be invited to the math test in February**
